Split / Referee Samples

Hold Harmless/Analytical Agreement Form

ISU RC offers secondary or split testing of samples that have tested positive for prohibited substances by other jurisdictional testing labs.  Please call Renee Novak for sample acceptance at 515-294-1210 or e-mail  rnovak@iastate.edu.

If you need your sample tested by a secondary laboratory you can contact us for verification of sample acceptance and pricing.  With most split testing the secondary laboratory must be on an approved list by your jurisdictional authority.  The jurisdictional authority will ship the sample.  All results will be reported to the jurisdictional authority within 10 working days after payment and a Hold Harmless/Analytical Agreement and payment are received.

Other steps to follow when submitting a split/referee sample:

• Contact our lab for sample acceptance
• Fill out and submit Hold Harmless/Analytical Agreement
• Submit Payment
  • No personal checks will be accepted
  • Payment allowed by bank’s cashiers check or credit card (Please call Chris Lee at 515-294-1950)
  • Split sample must be sent by next day priority mail, legally sealed with frozen ice packs (Ship M, T, W only)
• Sample testing will not commence until the payment and Hold Harmless/Analytical Agreement have been received.  (These must accompany sample or arrive prior to sample receipt for timely analysis.)

Sample testing will commence once everything is received.  We will make every effort to report results to jurisdictions within 10 working days after payment and Hold Harmless/Analytical Agreement are received.
