Orientation – May 14, 2012
CVM Research Day – August 10, 2012

Note:  Attendance on “Required Dates” listed above is mandatory for participation in the program. This program is a full-time stipend. Any vacation time beyond one week, or outside work which does not allow the participant to work full-time for this program, will not be allowed.

The mentor abstracts will be posted to the website by January 16, 2012. You will need to use this PDF to list your top three projects in this application. Your application is not considered complete without these rankings.

Please provide the names and day-time phone numbers of two references:

Brief description to include long-range professional goals; educational and personal experiences that you consider relevant to your career goals; benefits you hope to gain from the veterinary research scholar program; why this program interests you and what you will bring to the program.
Not to exceed 350 words.

Please include: Your student ID, work or lab experience, presentations and / or publications, organizations, honors and / or awards, and other personal interests or hobbies. Please be sure to include a section on Education and list undergraduate degree(s), undergraduate institution(s) attended, years attended, and year(s) degree(s) earned.
Also, include a valid email address and cell phone number.

Please note name of mentor and project if you have already established a mentor contact/relationship.
